Part 1: Creating a Fishbone Chart

  1. List an issue at your school that requires additional investigation.
  2. Using either the Word or the Excel template provided at  http://www.isixsigma.com/library/content/t000827.asp create a fishbone diagram of potential causes and effects.
  3. Send your diagram to your instructor.

Part 2: Developing an Action Plan

Based on the hypotheses outlined in your fishbone diagram, create an Action Plan that recommends Goals, Actions Required, Resources, and Indicators of Success.  Send your Action Plan to your instructor.

Using Data to Make Classroom Decisions

  1. State the question or questions you are planning to address. This should be classroom-related, and should involve at least two different data sources.
  2. List the data sources you will need to collect.
  3. Collect the data and organize it.
  4. Analyze the data and produce graphs to show patterns or trends.
  5. Write a synopsis of the action you plan to take.
  6. List "next steps" as the Plan, Do, Study, Act cycle begins again.
